The standard medical-and-surgical treatment of ovarian carcinoma patients relies on a systemic chemotherapy (carboplatinpaclitaxel), a tumor debulking surgery meant to be complete (no post-operative residual lesion), and a subsequent maintenance treatment with modern targeted agents. Recent studies identified a patient population (~14,000 patients / year in Europe), whose prognostic is poor (5 year-overall survival (OS) <20%) due to a refractory cancer, characterized by a poor chemosensitivity (assessable online with the numeric CA-125 KELIMTM score <1.0), and by a disease found non-resectable disease after 3-4 cycles of chemotherapy. In these patients, there is a high uncertainty about the best treatment adjustments to apply. SALVOVAR is a European project led by HCL, meant 1) to raise the physician awareness, and propose practical and affordable diagnostic tools for identifying these patients, and 2) to assess the utility (OS benefit), acceptability (quality-of-life; patient perception) and affordability (cost-effectiveness, including country coverage policies) of solutions based on adjustments of their medical-andsurgical treatment. These solutions implementable in routine may improve their prognosis, according to recent literature data. The project will be based on a large pragmatic randomized phase III trial, sponsored by ARCAGY-GINECO group, and activated in 6 countries (ENGOT network; ~100 recruiting centers), with the objective of demonstrating an OS benefit with the chemotherapy densification (weekly carboplatine-paclitaxel dose-dense regimen) compared to the continuation of the standard 3-weekly regimen. Total 685 patients treated with the standard neo-adjuvant chemotherapy will be pre-screened to randomize 240 patients. Dissemination, and communication will be carried-on to ensure the quality of the project, and inform the stakeholders, patients, public, health authorities/ payers of the project outcomes, mean to change the practices.

The e-Intervention Enhancing Mental Health in Adolescents project, IMPROVA, will co-design, pilot, evaluate, and facilitate the upscaling of a modular eHealth intervention platform that aims to improve mental health and well-being, early detect mental health problems and prevent common mental disorders in adolescents. The IMPROVA consortium includes an international and interdisciplinary group of researchers and practitioners from health, educational and social sciences in addition to computer scientists, a teacher association and policymakers. The IMPROVA online platform will be co-created with stakeholder groups, including adolescents, parents, teachers, school health professionals and policymakers based on materials already designed and tested in more than 20 projects carried out by the consortium members.
The platform will include components for adolescents, parents, teachers, and school health professionals in complementary and synergistic modules. After a series of pilot testing sessions, IMPROVA will be implemented by conducting a randomized Stepped Wedge Trial Design (SWTD) in secondary education schools randomly selected in four countries (France, Germany, Romania and Spain), including 12,800 adolescents. Effectiveness, cost-effectiveness and cost-benefit will be calculated. Using implementation science methodology, IMPROVA will co-design with policymakers and stakeholders transferable evidence-based practices, methodologies and guidance for upscaling of the IMPROVA platform. IMPROVA aims to provide stakeholders and policy makers with an evidence-based, innovative, large-scale, comprehensive intervention, and a scale-up plan to promote mental health and prevent mental disorders in adolescents; empower adolescents and families to make better decisions regarding their mental health; and provide schools and the community with tools to achieve a society with better mental health and lower stigma.

Esta es una propuesta bien escrita, excelente y competitiva que utiliza y amplía datos de la cohorte existente ENGNE. El estudio propone un seguimiento a los 48 meses para evaluar una cohorte de nacimiento longitudinal y analizar la relación entre experiencias prenatales y de la primera infancia con dificultades conductuales posteriores. La cohorte incluye mujeres embarazadas seguidas en el tiempo junto con sus hijos, recolectando datos de diversas muestras biológicas, medidas fetales y posnatales hasta los 48 meses, y exposición a factores ambientales y personales. Debido a la edad de la cohorte en crecimiento, este es un momento ideal para realizar el estudio. El tema encaja bien con la convocatoria de financiamiento, con objetivos innovadores y únicos a nivel global. La combinación de datos genéticos y ambientales es valiosa y merece seguir siendo financiada. El trabajo previo respalda la viabilidad del proyecto y la idoneidad del equipo de investigación. El plan de trabajo y el cronograma son claros y realistas, abordando vacíos científicos y necesidades de salud mediante un enfoque multidisciplinario que incluye neuropsicología, psicometría, genética, medio ambiente, función placentaria y neuroimagen. El equipo investigador es sólido y con experiencia en ENGNE, por lo que el proyecto probablemente tenga un alto impacto social y científico. Sin embargo, hay algunos puntos a mejorar: se espera que una minoría de la muestra tenga problemas conductuales o emocionales, pero el número de casos clínicamente significativos podría ser pequeño, lo que afectaría los análisis categóricos, por lo que se recomienda centrarse en medidas continuas para aumentar la potencia estadística. También hay un gran número de pruebas estadísticas propuestas, por lo que se sugiere enfocarse en preguntas más específicas e hipótesis dirigidas. Además, algunos análisis clave, como los de neuroimagen y función placentaria, se basan en una muestra reducida, lo que los hace susceptibles de estar subpotenciados. Finalmente, se deben abordar cuestiones como la migración, la edad de los niños al ingresar a la escuela, la inclusión de hermanos, cambios en la estructura familiar y la evolución de los entornos sociales.
